COVID-19 Vaccine Distribution at Story Medical

Story Medical offers COVID vaccine appointments for all eligible patients through MyUnityPoint (see instructions below). You can also schedule by calling Story Medical Clinic in Nevada at (515) 382-5413 or Maxwell at (515) 387-8815. We are offering appointments daily and they are required for all vaccines. Please note, anyone under the age of 18 must have a parent present to provide consent prior to receiving the vaccine.  

COVID-19 Testing at Story Medical

Scheduling Your Vaccine through MyUnityPoint

Current Story Medical patients can view available appointment times at www.chart.myunitypoint.org. If patients do not currently have a MyUnityPoint account, they can create one at https://chart.myunitypoint.org/MyChart/signup.
